** If you elected to participate in the HEM program, to access the HEM Personal Health Assessment and watch the 2 required e-lessons (Providence calls these Health Converstations) go the myProvidence or Kaiser healthworks website. You will need to create your account using your medical insurance id card. If you have a spouse or domestic partner enrolled in medical insurance, they will need to complete the requrements by creating their own account. Additional information:

Life Changes - Information on losing coverage or making changes to your current coverage.

Did you have a baby or adopt a child, get married or divorced, change to part-time or full-time employment or move across the state? Did your 19 year old go to college, or your 23 year old move back home? Did your employment terminate, go on leave without pay or have a reduction in your hours?

Your Public Employees' Benefit Board (PEBB) gives you ways to deal with these and other life changes that happen after open enrollment. The PEBB rules give you 30 days from the date of an eligible event to make related changes to your benefits.
